We greatly value our relationships with the dental professionals and families who trust us with their orthodontic care. Whether you are a doctor who would like to refer a patient or you are interested in scheduling an appointment for yourself or your child, we are here for you! Just fill out our brief online referral form and we will take care of the rest.

Referring Doctors

We now offer two online referral options. Please select your preferred referral type below. If printing, please scan a copy and email it to our office at newpatients@kandrortho.com. If you are submitting a digital referral it will be sent to us automatically.

Please select an option below to be taken to our New Patient Registration forms. Once the forms have been completed our office will be notified, create your chart, and then reach out to schedule your complimentary consultation.
